Newer No-Mask Therapies

 

Continuous positive airway pressure (CPAP) masks can be cumbersome, which is a big reason some people struggle with them.  But more options are becoming available.  Here are newer treatments that don't require a mask during sleep:

  • Expiratory positive airway pressure (EPAP) :This comes in the form of a single-use device that is placed over each nostril before you go to sleep.  The device is a valve that allows air to move freely in, but when you exhale, air must go through small holes in the valve.  This increases pressure in the airway and keeps it open.  In a trial, EPAP nasal devices significantly reduced the number of breathing disturbances and improved daytime sleepiness.
  • Negative presure therapy: the Winx Sleep Therapy system provides negative pressure by gently holding the tongue forward during sleep.  This is achieved through the use of a semicustom fit mouthpiece with a small tube attached.  Suction holds the tongue forward, preventing it from blocking the airway.  
  • Implantable Devices:  If you can't tolerate CPAP, you may be a candidate for a hypoglossal nerve stimulation device (Inspire).  This remote control devise is much like a pacemaker.  It's implanted just under the skin and a wire is attached to one of the hypoglossal nerves that control tongue movement.  Stimulation of the nerve is coordinated with your breathing, to move your tongue slightly forward in your mouth when you breathe, and as a result avoids obstruction.

Left untreated, nighttime breathing disorders not only cause daytime sleepiness and fatigue, but also can lead to cardiovascular problems.  Once the condition is identified, treatment can be very effective, with therapies ranging from simple lifestyle changes to specialized breathing devices, dental appliances surgery or even an implantable device.

  • Pauses in breathing during sleep
  • Snoring that's loud enough to disturb another person's sleep
  • Excessive daytime drowsiness, which may cause you to fall asleep while you're working, watching television or driving.
